Changing your Contact Address
JRVS member information is computerized based on the member's card submitted at the time of admission.
Based on the information, various announcements and General Meeting abstract collections are delivered and the membership list is created.
If any changes arise as described below, fill out the Notice of Change (Form 1) and send it to the secretariat via postal mail or fax as soon as possible.
- Change of name
- Change of place of employment (position, postal code, address, phone number)
- Change of residence address (postal code, address, phone number)
- Change of address (business address or residence address) for receiving JRVS journals (General Meeting records).
Note:
If your residence address changes as a result of a change in your business address (or vice versa), please make sure to report both of the new addresses.

About the Notice of Withdrawal
Fill out the Notice of Withdrawal (Form 2) and send it to the secretariat via postal mail or fax.
